What’s wrong (safety wise) in the picture of the two men working on the flat roof?

Lanyards aren’t attached to anything. (Although the workers have personal fall arrest equipment on, if it isn’t hooked up to a secure anchor point capable of supporting 5000 pounds per employee, it’s the same as not having fall protection).

No guardrail system or safety net system. (A guardrail or safety net system could be used to prevent falls from the roof).
